Thanks Hillary for getting this done so quickly, I hope others will come to you if they’re having any troubles creating or embedding the form.
How did you get the form to show up as it’s picture? When I copy my link to embed it, only the link shows up.. :S
To get it so the picture shows and not the link, you have to put the Embedded code in the box not the link of the site.. After you click “save” when you finish your survey, right beside the save button will be a button that says “more actions”. Click on that and a scroll down should appear. Click the one that says “Embedded” a box will pop up with a link inside it, that’s the link/code you will put in your blog entry!
